# Why Pay Privately for Psychiatry When Your Family Already Has Health Insurance?

![Dr. Allison J. McDade, MD](https://www.healthandharmonypsychiatry.com/hs-fs/hubfs/dr-allison-mcdade-author.webp?width=48&height=48&name=dr-allison-mcdade-author.webp)

 Dr. Allison J. McDade, MD

- Medically reviewed November 2, 2026
- Updated November 2, 2026

Maybe you've sat at the kitchen table after everyone else is asleep, adding up what your family already pays for health insurance. Paying a psychiatrist on your own can feel like paying twice.

Or maybe you've used your plan before and run into a deductible you didn't plan for, or a plan change that meant a new doctor and your story told again from the start.

And maybe your own care keeps landing at the bottom of the list, after everyone else's.

You deserve clear numbers, and a physician with time for your whole story.

This guide lays out the numbers, so the choice is yours.

The answer

**Private pay can make sense when time with one physician matters most to your family.** Your health plan stays in place for the rest of your care. At Health & Harmony, you pay for each visit with Dr. McDade directly, at two posted rates. With your plan, you may pay the full allowed amount for each covered visit until your deductible is met. Many plans pay for some services sooner, so ask yours. **Know your numbers first, then choose what fits your family.**

Her story explains [why Dr. McDade built a different model of care](https://www.healthandharmonypsychiatry.com/why-a-different-model).

## What does your family's insurance pay for?

Your plan pays for covered care by its own rules. The premium is the plan's monthly price, and with a plan through a job, you and your employer share it. On top of it, you usually pay a deductible, copays, and coinsurance, HealthCare.gov explains.

The most you pay for covered in-network careYour out-of-pocket limit

Paid firstYour deductibleWhat you pay for covered care before your plan starts to pay

Paid afterCopays and coinsuranceA set amount or a share of each covered service, while your plan pays the rest

Once you reach the limit, your plan pays the full cost of covered in-network care for the rest of the plan year. Adapted from HealthCare.gov's glossary.

Your plan also has a network, the providers it has contracted with, and an allowed amount, the most it will pay for a covered service.

**Your premium buys coverage. For most care, you pay your deductible first.**

## Why can a family with insurance still pay the full cost of a visit?

Because of the deductible. Before you meet it, you may pay for a covered visit yourself, at your plan's allowed amount. After you meet it, you usually pay a copay or coinsurance, and your plan pays the rest.

What you pay for a $100 doctor visit, in HealthCare.gov's examples

- Before your deductible is met$100
- After it is met, with a $20 copay$20
- After it is met, with 20% coinsurance$20

From HealthCare.gov's glossary, where the plan allows $100 for the visit. Your plan's amounts and rules may differ.

In 2026, 88% of workers with single coverage had a deductible that must be met before most services are paid for, KFF found. Many plans pay for some services sooner, like a checkup, HealthCare.gov notes.

**Until your deductible is met, a covered visit can cost the full allowed amount.**

## What does a family plan through a job cost in 2026?

Family coverage through a job averaged $28,303 a year in premiums in 2026, KFF's survey found. That counts what you and your employer pay together. The most common kind of plan is a preferred provider organization (PPO). A PPO lets you see providers outside its network for an additional cost, HealthCare.gov explains.

- $28,303The average yearly premium for family coverage, you and your employer together
- $7,471What workers paid each year, on average, toward family coverage
- 50%Of covered workers are in a PPO, the most common kind of plan

From KFF's 2026 Employer Health Benefits Survey. Both dollar figures average family plans of every type.

Those are real dollars, and it makes sense to want them to work for you. Each paycheck covers part of that premium. Family plans often have a deductible for each person and one for the whole family, HealthCare.gov notes. Your plan can tell you what is left on each, and its allowed amount for a [psychiatric evaluation](https://www.healthandharmonypsychiatry.com/glossary#psychiatric-evaluation) and a follow-up visit.

**The allowed amount is the number to ask your plan for.**

## What does private pay make possible here?

Time with one physician. Private pay, also called self-pay or cash pay, means you pay the practice directly for each visit. Dr. McDade sees every patient herself, because that is the work she trained for.

Your first visit needs time, space, and connection, so she sets aside room for your whole story. That first evaluation looks at your medical history, your symptoms, and the stress in your life right now. So bring what seems unrelated, like sleep, a recent illness, or a hard season at work. After that, your story carries forward, and each follow-up builds on what she already knows.

If you fill a prescription through your health plan, the plan's rules still apply, and some prescriptions need its approval first. When one does, Dr. McDade calls and helps however she can, as part of her standard fees.

**One physician, with time for your whole story.**

If you need help now

If you are in crisis or thinking about suicide, call or text 988 or chat at [988lifeline.org](https://988lifeline.org/) to reach the 988 Suicide and Crisis Lifeline. If you are Deaf or hard of hearing, use your preferred relay service or dial 711, then 988. In an emergency, call 911 or go to the nearest emergency room. This practice does not provide emergency services.

You've done the math for everyone. Now the time is yours.

## When does using your plan make more sense?

When your plan already meets your needs. Both paths are reasonable.

HealthCare.gov notes two limits on care outside your network. It doesn't count toward your plan's in-network out-of-pocket limit. And health maintenance organization (HMO) and exclusive provider organization (EPO) plans generally cover it only in an emergency.

Your plan may fit better when

- Your deductible is met, or nearly met
- A psychiatrist in your network suits you and is taking new patients
- You want each visit to count toward your in-network out-of-pocket limit

Private pay may fit better when

- Time with one physician matters most to you
- You want the same physician through a job or plan change
- You want a posted price for every visit

You can also use both, your plan for your family's other care and private pay for your time with Dr. McDade.

**Choose the path that fits what matters most to your family.**

## What should you ask your plan before you decide?

These five questions fill in the numbers this guide leaves blank. Set the answers beside the two posted rates below.

Questions to ask your plan first

- Which psychiatrists in your network are taking new patients?
- What is my deductible, and how much of it is left this year?
- Is a visit with a psychiatrist covered with a copay before my deductible is met?
- What do you allow for a psychiatric evaluation and a follow-up visit?
- Do you offer out-of-network mental health benefits?

If your plan fits better, start with its directory of psychiatrists.

**A fair choice starts with your plan's own answers.**

## Can you use an HSA or FSA?

Yes. You can pay for your visits with a card from a health savings account (HSA) or a flexible spending account (FSA). Psychiatric care counts as a medical expense under IRS Publication 502.

FSAs are generally use-it-or-lose-it plans, the IRS notes. Ask your account's provider what it covers, and keep each receipt.

If your plan pays you back

Tell your account's provider and ask what to do. HSA money is tax free only for medical costs insurance hasn't paid. Other uses are taxed and may carry an extra tax, IRS Publication 969 explains. An FSA pays only for costs no other health plan covers.

**Money you set aside for health care can go to your own care.**

## What do the fees cover?

Each fee covers a [telepsychiatry](https://www.healthandharmonypsychiatry.com/glossary#telepsychiatry) visit with Dr. McDade, by secure video, for patients in Houston and across Texas. Coordination with your other clinicians, once you sign a release, comes with your care.

## Questions about paying privately

How much does care cost?

Care is private pay. A new evaluation with Dr. McDade is $465, and follow-up visits with Dr. McDade are $275. You receive a good faith estimate before your first visit.

Does Dr. McDade take insurance?

No. Care is private pay, with rates set in advance and shared up front. You have the right to receive a [Good Faith Estimate](https://www.healthandharmonypsychiatry.com/good-faith-estimate) explaining how much your medical care will cost. Under the law, health care providers need to give patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services.

Do private-pay visits count toward my deductible?

It depends on your plan, so ask it first. The practice can give you a superbill to send to your plan if it offers out-of-network mental health benefits. It lists the visit, its billing codes, and what you paid. Your plan then decides what counts toward your deductible and what it pays back, from its own allowed amount. If a fee is more than that amount, you may have to pay the difference, HealthCare.gov explains.

Can I pay with my HSA or FSA?

Yes. The practice takes HSA and FSA cards, and psychiatric care counts as a medical expense under IRS Publication 502.

Can my child see Dr. McDade too?

Yes. You and your child can both be her patients, and each of you can be understood. She is board certified by the American Board of Psychiatry and Neurology in psychiatry and in child and adolescent psychiatry. She sees children from age 6, and a parent or guardian books for anyone under 18.

Will I see the same doctor at every visit?

Yes. Health & Harmony Psychiatry is a physician-only practice, and every visit, from your first evaluation through every follow-up, is with Dr. McDade.
