Good Faith Estimate

Your right to a Good Faith Estimate
If you do not have insurance, or you choose not to use insurance, you have the right to know what your care will cost before you receive it. Health & Harmony Psychiatry provides a Good Faith Estimate to every uninsured and self-pay patient.
You will receive a Good Faith Estimate in writing before your visit. You can also request one at any time, for any service, by contacting us at Admin@healthandharmonypsychiatry.com or 832-387-6088.
Notice under the No Surprises Act
You have the right to receive a Good Faith Estimate explaining how much your medical care will cost.
Under the law, health care providers need to give patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees.
Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your medical service or item. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
Make sure to save a copy or picture of your Good Faith Estimate.

How this works at our practice
Health & Harmony Psychiatry is a cash-pay practice, and we also accept Aetna. Our standard self-pay fees are:
- New psychiatric evaluation with Dr. McDade: $465
- Follow-up visit with Dr. McDade: $275
- Follow-up visit with our physician assistant: $195
Your written Good Faith Estimate lists the expected charges for your evaluation and ongoing visits, based on the care you and Dr. McDade plan together. An estimate is not a bill, and your final cost may change if your care changes. We will talk with you before that happens, and you can ask questions about your estimate at any time.
