The short version
We collect what we need to answer you, run this website, and care for you, and we do not sell your personal information. Health information belongs in the secure patient portal, never in email or a website form. Texas law adds protections of its own, set out in the Texas section below.
What this policy covers
This policy covers the information you share through this website, by email or phone, and when you book a visit.
Health information you share during clinical care is protected separately under federal and state law, including HIPAA, and is described in our Notice of Privacy Practices, which you receive when you become a patient.
As part of your care, we may use trusted third-party services to help document and manage your clinical records, including services that use artificial intelligence to assist with visit notes. This protected health information is handled under HIPAA and under business associate agreements, and is described further in our Notice of Privacy Practices. This website policy does not replace that notice.
What information we collect
When you contact us or book a visit, you may give us your name, email address, phone number, and what you write to us.
When you visit, this website and the tools it uses may collect technical details, such as your device, browser, general location, and the pages you view.
Keep health details in the portal
Please do not send health information through a website form or by email. Current patients share clinical details through the secure patient portal.
How we use and share information
We use your information to respond to you, schedule visits, run and improve this website, and meet our legal duties.
We do not sell your personal information. We may share information with trusted service providers who help us run the practice and the website, such as our scheduling and records platform, website hosting, and analytics, under appropriate safeguards. We may also share information when required by law.
Cookies and outside tools
This website uses cookies and similar tools to keep it working, measure traffic, and improve the experience.
Some of these tools come from other companies. HubSpot hosts this website, and HubSpot and Google Analytics measure its traffic. Google Fonts serves its fonts, and CharmTracker runs the booking calendar.

How we protect information
Your clinical records are kept in CharmTracker, the practice's secure records system and patient portal.
We use reasonable measures to protect the information we hold. No website or email system can guarantee absolute security.
This website links to other sites, such as the patient portal and professional profiles, and their own privacy policies apply there.
Your records and your choices
You can ask to see the information we hold about you, ask us to correct it, or ask us to delete what you gave us through this website. You can also stop non-essential messages from us at any time.
After written notice, records are copied within one week for pickup or mailed to the address on file.
To make any of these requests, contact us using the details at the end of this policy.
Children and teens
This website is intended for adults. It is not directed to children, and we do not knowingly collect personal information from children through this site.
Care begins at age 6, and a parent or guardian books for anyone under 18.
Privacy in Texas
Texas law adds its own protections for health information, in Chapter 181 of the Texas Health and Safety Code.
Notice of electronic disclosure
Texas law requires us to tell you that your health information may be shared electronically. We share it electronically only with your authorization, except with another covered entity for treatment, payment, or health care operations, or when state or federal law allows or requires it.
If you ask in writing for your electronic health record, Texas law requires us to provide it in electronic form within 15 business days, unless you agree to another form.

We may update this policy from time to time. When we do, we will post the new version on this page and revise the date at the top.
